In 1990, Salt Lake City dentist, Dr. Ralph Montgomery, recognized access to oral health care for the disadvantaged within the community was nearly nonexistent. Many of the poorest and most destitute were forced to go without needed dental care. Often restorable teeth were extracted because there were simply no other options available. As a result, Dr. Montgomery founded Salt Lake Donated Dental Services (SLDDS).
Salt Lake Donated Dental Services offers quality, comprehensive dental care to low-income and homeless individuals. Their dental professionals provide patients with treatment options comparable to what they would expect from a private office. SLDDS treats all patients regardless of ability to pay. Donated and discounted services are available depending upon family size and income. Printed proof of income eligibility such as most recent pay stubs or Social Security statement is required at every visit.
