To be a nationally recognized health and wellness corridor known for comprehensive patientcentered services, it's highly qualified and dedicated team of health professionals, advanced technology and collaborative partnerships. Since 1978, Charter Oak Health Center, Inc., a federally qualified, freestanding, Joint Commission accredited, nonprofit community health center – has continuously provided community-based primary health care to residents in the greater Hartford area. We have grown from a small organization operating out of an apartment building to the 60,000 square foot building our patients now call their medical home.
Charter Oak Health Center has received the Gold Seal of Approval by the Joint Commission, an independent, notforprofit organization. The Joint Commission accredits and certifies more than 20,000 health care organizations and programs in the United States. Joint Commission accreditation and certification is recognized nationwide as a symbol of quality that reflects an organizations commitment to meeting certain performance standards. Charter Oak Health Center receives funding from HHS and has recognized Federal Public Health Service (PHS) status for itself and for individuals for certain health or health-related claims, including medical malpractice claims.
