Six months ago, Sakuta had a life-changing encounter with a girl dressed as a bunny in a library. Since that moment, he's been in a blissful relationship with Mai Sakurajima - who was that same bunny girl! However, things take an unexpected turn when his mysterious first crush, Shouko Makinohara, resurfaces as an adult. This unexpected reunion adds a new layer of complexity to Sakuta's relationship with Mai.

To further complicate matters in "Seishun Buta Yarou wa Yumemiru Shoujo no Yume wo Minai," Sakuta encounters a middle school-aged Shouko, who is now hospitalized and grappling with a serious illness.
